Below is an ActiveX Control built using Microsoft Visual Basic 6.0. The control took about 2 hours to code. The Setup and Deployment Wizard walked me through the creating of a .cab file and even created a sample HTML page for me.

Instructions: Step 1 - Select a base number Step 2 - Select an order for the questions to be asked in Step 3 - Select Addition, Subtraction(not completed) Multiplication or Division Step 4 - Click "Go" Step 5 - Answer the questions as fast as you can, you are getting timed Note: as you enter the answers, just hit enter, the next button is the default for the form.
